Transforming Care for the Future
Here in Transformation we are working to
support the delivery of the Transforming Care for the Future
strategy, which was agreed by the Trust Board in April
We recognise there has been some fantastic work done in
the Trust to make things better for patients and staff, so we are
here to build on this and support this work. We are also leading on
some separate key pieces of work, which we can't do on our own - we
need staff involvement to help drive the changes and help us reach
our targets! The overall aim of the strategy is simple but
ambitious - we want CMFT to be in the top 10% of similar sized
Trusts with similar complexities for quality. Quality includes
patient outcomes, patient safety, both patient and staff
experience, as well as operational efficiency.
Transformation Vision, Values and
Transforming Care for the Future is here to support and
facilitate change in the Trust.
We will build on existing change projects and support and
develop staff who have a passion for making things better for
patients with the resources available. We will bring and share best
practice in order to maintain and drive improvements to make us the
best we can be, creating a culture where people feel able and
confident to think outside the box.
We will do this through…
- Providing cutting edge care through the use of horizon
scanning, seamless care and integration, improved partnership
working and further development of networks.
- Skilling up our leaders; both clinical and non-clinical,
developing consistent improvement techniques and empowering staff
at all levels to be involved in change.
- Ensure learning and best practice is shared and create support
- Delivering large scale change, which will be reported to Trust
Board and will help to build the Trust reputation
